How to Complete the ITR-DD Form

 The ITR-DD form confirms a taxpayer’s disability for SARS purposes.
Even small errors can delay or invalidate your claim — this guide shows you how to complete it correctly the first time. 

Step-By-Step Guide

Step 1: Complete All Required Fields

All required fields in Part A must be completed. This will generate a Document Unique Number on the last page of the document.

SARS will not process the form unless it is fully completed and the Document Unique Number is generated.

Step 2: Use the Correct Medical Practitioner

The form must be completed by a practitioner qualified to diagnose the specific condition.

Refer to the ITR-DD guidelines to ensure the correct practitioner is used.


Using the wrong practitioner can result in your claim being rejected.

Step 3: Use the Correct Date of Determination

This is the date the patient first met the criteria for the disability.


It is NOT:


  • The diagnosis date 
  • The date the form was completed 
  • The date the doctor signed 


This is one of the most common mistakes.

Step 4: Ensure the Doctor Signs and Stamps the Form

The form must include:


  • The doctor’s signature 
  • The official practice stamp 


Missing stamps are one of the most common reasons for SARS rejecting claims.

Step 5: Final Checklist Before Submitting to SARS

 Before submitting your ITR-DD form, make sure:


  • All fields are completed (no blanks) 
  • The form is signed by the taxpayer
  • The correct practitioner has signed 
  • The Date of Determination is accurate 
  • The form includes both signature and stamp 


Missing any of these can delay or invalidate your claim.
