What is the ITR-DD form?

 The ITR-DD is a form completed by a registered medical practitioner to confirm a qualifying disability for tax purposes under Section 6B in South Africa.


Even small errors can delay or invalidate your claim.  


This guide shows you how to complete it correctly the first time. 

Step-By-Step Guide

Step 1: Download the latest version of the ITR-DD

Complete the form in your Internet Browser

Step 2: Complete All Required Fields

All required fields in Part A must be completed. This will generate a Document Unique Number on the last page of the document. SARS will not process the form unless it is fully completed and the Document Unique Number is generated.

Step 3: Use the Correct Registered Medical Practitioner

The form must be completed by a registered medical practitioner qualified to diagnose the specific condition.

Refer to the ITR-DD completion guidelines to ensure the correct practitioner is used.

Using the wrong practitioner can result in your claim being rejected.

Step 4: Use the Correct Date of Determination

This is the date the patient first met the criteria for the disability.


It is NOT:

  • The diagnosis date 
  • The date the form was completed 
  • The date the doctor signed the form

Step 5: Ensure the Doctor Signs and Stamps the Form

The form must include the doctor’s signature and the official practice stamp

The ITR-DD form  must be completed by taxpayers who have a physical, sensory, communication, intellectual, or mental impairment, or whose spouse or child has a disability.

  • Part A: Completed by the taxpayer or their guardian.
  • Parts B, C, and D: Completed by a registered medical practitioner trained to diagnose the qualifying disability under Section 6B of the Income Tax Act.


There is no separate registration or submission process for the ITR-DD.  Submit the completed ITR-DD form to SARS by uploading it as a supporting document via eFiling or the SARS MobiApp when submitting your annual income tax return (ITR12). Keep the original, doctor-signed form for your records, as it is only submitted if requested for audit.


An ITR-DD is generally valid for 10 years if the disability is permanent and first occurred on or after March 1, 2019. For temporary disabilities, the form is valid for one year and must be renewed annually. 


 The following mistakes can lead to SARS rejecting your disablity tax claim:

  • Completed by incorrect registered medical practitioner
  • Incorrect classification of impairment 
  • Vague or non-specific diagnoses & medical information
  • Incorrect Date of Determination
